Form W-2: Printing on perforated paper

• Purchase enough perforated paper for your employees. You'll need at least two pages for each employee. Form W-2: Where can I purchase perforated paper for printing employee copies?

If you don't have enough perforated paper, you can print the page with Copy B, 2, and C on perforated paper and the page with just Copy 2 on plain paper by inserting plain paper between the perforated pages.

• In special cases, some employees have more data and need more than two pages. For example, if an employee is subject to more than two local taxes, or has more than two items in Box 14, or more than four items in Box 12, we print additional copies of Form W-2 with just the additional data.
