Forms: Filing amended forms

The amended form basically documents the differences between the original and updated versions of the original forms.

To file an amended form:

  1. Get a blank amended form from the tax agency.
  2. Get the original and updated versions of the form:
    1. The original version is available on the Archived Quarterly Forms or Archived Annual Forms page.
    2. The updated version is available on the “actual” Quarterly Forms or Annual Forms page.
  3. Fill out the blank amended form using the original and updated versions.

An exception is federal Form 940. You can print a corrected Form 940, mark the “Amended” checkbox in the top right corner, and mail it to the IRS.
